Success in this role is defined by a 4.8/5 Patient Satisfaction Rate (CMSR). You will be successful when you can seamlessly pivot from performing a precision clinical assessment in a patient's living room to facilitating a flawless 4K video consultation with a specialist. You are a successful Care Manager when you capture the "heart" of recovery through storytelling while maintaining 100% accuracy in digital medical records.

Organizational Fit

The Care Manager reports directly to the Clinical Director/Operations Manager. You are the "face" of the company in the field. While our tech provides the platform, you provide the trust. Your insights from the field directly inform our product development and our brand’s social media narrative, making you a core driver of Careline’s growth in the Karachi market.

Responsibilities

• In-Home Clinical Leadership: Execute professional home visits to monitor health status, manage complex medication schedules, and provide early intervention assessments to prevent hospitalizations.
• Tele-Health Facilitation: Act as the "technical hands" for physicians during remote consultations, setting up digital interfaces and ensuring a seamless experience for both doctor and patient.
• Precision Documentation: Manage and update Electronic Health Records (EHR) with zero margin for error, ensuring all real-time medical data and appointment scheduling are perfectly synchronized.
• Content & Storytelling: Collaborate with patients (with documented consent) to capture high-quality photos and short-form video content that highlights the "Careline Experience" for our digital platforms.
• Patient Advocacy & Communication: Serve as the dedicated point of contact for families, providing empathetic support and clear communication in both English and Urdu.

Qualifications

• Educational Background: Valid Nursing Degree (RN), Medical Assistant certification, or a related healthcare diploma.
• Tech-Proficiency: Highly comfortable navigating EHR systems, tele-conferencing tools (Zoom/Google Meet), and smartphone camera technology.
• Communication Excellence: Fluent in English and Urdu; ability to simplify complex medical information for elderly patients and their families.
• The "Careline Personality": Exceptional empathy, patience, and a "heart-first" approach to caregiving.
• Operational Requirements: Valid driver’s license and access to reliable transport for frequent home visits within DHA and surrounding Karachi areas.
• Attention to Detail: A proven track record of maintaining meticulous medical records and never missing a follow-up.
